You are responsible for self-certification for the first seven days of any illness including Sundays and your employer should provide you with the form SC2. Patients who are self-employed should complete the form SC1 that is available from the DSS. Thereafter you will need a doctor’s note (Fit Note). You will usually have to see a Doctor to arrange a first Fit Note and you can phone into the surgery to arrange further requests. Review appointments will be needed from time to time. Please allow sufficeint time for ordering continuation lines.
Requests for sick lines do not constitute medical emergencies.
